One of the roles of our Southampton team is 'Transition Links'. We provide advice and support for services which work with individuals who have a learning disability (and who will require adapted and specialist learning disability health support from 18 years old, whose health needs can not solely be met by primary care).

Find out more by contacting the team to get a copy of the Southampton City Transition to Adulthood - Practice guide.
