The Role of the Health Facilitator

The Health Facilitators work in Hampshire and Southampton are Learning Disability Nurses who have a dedicated role to support professionals by:

  • D & M Photography-24.jpgProviding learning disability awareness training to outside agencies, including sharing information on learning disability services and the support available

  • Providing advice on reasonable adjustments to primary and secondary healthcare services

  • Ensuring that the learning disability population is included on local Clinical Commissioning Group (CCG) and Public Health (PH) agendas

  • Ensuring that national screening programmes consider the needs of people with a learning disability

  • Developing pathways with generic services that meet the needs of people with a learning disability

  • Providing training on learning disability enhanced services and the delivery of annual health checks for GPs and practice staff

  • Supporting general practices to establish and maintain a Learning Disability Register

  • Discussing and advising on missed health appointments

  • Promoting and sharing useful resources e.g. a Hospital Passport (also know as 'All about me' book) with people with learning disabilities, their families and carers, as well as primary and secondary health services and other agencies.
