The Quality Improvement (QI) project focussing on integrated discharge from Kingsley Ward at Melbury Lodge recently had its 90 day review where the group met to discuss their progress.
The project identified four areas to focus on: opportunities on admission, ‘supporting’ processes, rehabilitation and the ‘funding’ panel.
The improvements achieved, included: the redesign of the rehabilitation pathway, the reduction of non-value added activity for staff, the streamlining of the application process for additional funded care and the introduction of ‘Dorothy’s Drop-Ins.’ The drop-ins enable people to get support with housing and benefits to promote discharge from hospital in a timely manner. The group are planning to meet again in six months and then a year to further review their progress.
