Lifestyle, Health and Wellbeing Survey (HWB)


The aim of this survey is to provide information about the health and wellbeing of people with SMI. The two main objectives are: To benchmark current health related behaviours of people with severe mental ill health; and to provide a platform for future research with this population.

What is involved?

You will be asked to complete a short survey, which will take approximately 5 minutes to complete. This will then be posted in a free post envelope back to the University of York research team.


To take part you must meet the following criteria:

  • Aged 18 years or over

  • Have a recorded diagnosis of a serious mental illness (SMI) including schizophrenia, schizoaffective disorder, bipolar affective disorder and associated disorders


Unfortunately, you cannot take part if:

  • You are a non-english speaker

  • OR lack the mental capacity to give consent to participating
