Improving transitions into the Community Learning Disability Health Service

Transition into learning disability health services is the gradual preparation and transfer from children to adult health services.  Every young person has different health needs, and therefore their journey through transition will differ.

At Southern Health NHS Foundation Trust, we understand that it is important for young people (and their family/carers) to have a positive experience of transition into adult health service.

Community Learning Disability Services

Adult health services differ to children’s services. They are subject to eligibility criteria and having specific health need(s), to ensure that people have support from the right service when they need it. For most young people, their health needs will be met by their GP.  At times they may require support from a specialist and adapted learning disability health service.

As a Trust, we are always keen to improve the services and support we offer to people, particularly for those in the process of transition, which we understand is often a challenging time for young adults and those who support them.
