Community Nursing IOW image.jpgThe Community Nursing Service offers an open referral system allowing people (or their carers) to self-refer and health/social care professionals to refer a person for assessment if they meet the service entry criteria of being housebound.

 

What we do

We support people who need skilled nursing interventions who might be unable to leave their home or whose treatment is more appropriately delivered at home.

Accessing the service

The Community Nursing Service offers an open referral system allowing people (or their carers) to self-refer and health/social care professionals to refer a person for assessment and where appropriate Continuing Healthcare provision. You can do this by calling the team using the numbers below or by filling out the referral form.

Community Nursing is available from 8am to 8pm, 7 days a week. After 4pm, during, weekends and bank holidays – Call 111At All Other Times Contact the Community Nursing Teams.

The team operate out of three main bases:

West and Central Locality
Covering Cowes, Newport, West Wight areas
Newport Team based at The Health and Wellbeing Centre, Newport (opening April 24)

North East Locality
Covering Ryde, East Cowes, Bembridge area
Based in Ryde Health & Wellbeing Centre 

South Wight Locality
Covering Sandown, Shanklin, Ventnor, Chale areas
Based at The Barracks, Sandown
