Transforming our Services: Older People's Mental Health

We have identified areas for improvement for us to focus on: 

Supporting patients with organic mental illness (dementia, memory problems)
Developing our memory assessment service, improving how we support patients with challenging behaviour at home and in care homes, and achieving national accreditation for all memory services. 
 

Supporting patients with functional mental illness (anxiety, depression and psychosis) 
What we can do to support people at home rather than admitting them to hospital. How we can offer access to mental health care for older people.  

Inpatient facilities 
Determining how many beds we need in the future and how people access our older people's mental health inpatient facilities. 

Access to psychological therapies in OPMH
Enabling improved access to psychological therapies to older people in the community and our hospitals, including access to italk.

Deploying new ways of working, developing new roles to provide the serivces and care our patients need. 

Memory Assessment Clinics

The number of people accessing our memory assessment services is on the increase due to a rising ageing population and more people living with dementia. In order to meet the growing demand, we want to review our services to understand if and how we could make them better. Ultimately, we want to ensure the experience for those people accessing the service is a positive one.

Intensive Support Team

Short term mental health support to keep our patients in their own home and avoid an unnecessary admission to hospital and offer support for those being discharged from hospital. Currently operating in the Portsmouth and South East Hampshire area.
