Community Mental Health Teams

We provide a range of community-based mental health treatments and therapies for people who need support for their mental health issues. These include:

  • prescribing and managing medication

  • psychological (talking) therapies

  • occupational therapy

  • specialist support for psychosis

  • assertive outreach for people who struggle to keep in contact with their mental health services

We also provide advice to families and carers to help them support loved ones.  (We encourage patients to involve family and carers in their treatment, but this is their choice. We’ll only disclose information with a patient’s consent or in exceptional circumstances when their safety may be at risk).

Importantly, we encourage individuals to take greater control over the way they’re treated, setting their own goals, priorities and hopes for the future.

Accessing our services

If you feel that you need support from any of our services, speak to your GP to request a referral. Your local CMHT team will then do a thorough assessment with you to make sure you’re signposted to the right services. Together with you, we will create a care plan to support you in the best way possible.

What our patients say…

"The consultant I saw spent a lot of time with me, truly listened and gave me a number of options for treatment, the team was careful to make sure I was comfortable with the decision I made."

"This service is a valuable resource to someone with similar problems to me...the team explained things, listened to me, and were considerate to my feelings."

"Thank you for all your care and support during my difficult times - you have given me my life back."
