We also provide advice to families and carers to help them support loved ones. (We encourage patients to involve family and carers in their treatment, but this is their choice. We’ll only disclose information with a patient’s consent or in exceptional circumstances when their safety may be at risk).
Importantly, we encourage individuals to take greater control over the way they’re treated, setting their own goals, priorities and hopes for the future.
Accessing our services
If you feel that you need support from any of our services, speak to your GP to request a referral. Your local CMHT team will then do a thorough assessment with you to make sure you’re signposted to the right services. Together with you, we will create a care plan to support you in the best way possible.
