Who are we?
We are a team who provide education and support to patients, GPs and other health professionals in the South East of Hampshire.
As a non case-holding team we do not receive referrals but we receive requests for clinical guidance and education.
The South East Community Diabetes service consists of Diabetes Specialist nurses, Coordinators, Desmond lay Educators and Administrators .
We also work closely with Portsmouth Hospitals Trust.
What do we do?
We run courses for people with type 2 diabetes across the South East of Hampshire. These courses can help you to learn more about your condition and manage it more effectively. It’s also a great opportunity to meet other people with diabetes and to ask questions or voice concerns.
Diabetes Education Self Management On Going for Newly Diagnosed, is aimed at people in the first year of diagnosis of type 2 diabetes.
Desmond is a 1 day course which helps you find out more about type 2 diabetes, activities and lifestyle adjustments you can follow help to manage your condition.
Usually a small group of people with their partner or family member or friend held at a local venue over one day.
The day is led by our team who are trained to deliver you with honest, up to date, evidence based information to give you options to manage your diabetes.
There is a lot of conflicting, out-of-date information available. This is your opportunity to acquire an understanding of Type 2 Diabetes and a chance to discuss and explore factors such as food choices, activity and medication.

Type 2 diabetes refresher sessions
We provide a 4 hour course aimed at people with type 2 diabetes who have either been diagnosed for longer that 12 months or feel they need fresh input into managing their condition.
