South East Community Diabetes Service

Who are we?
We are a team who provide education and support to patients, GPs and other health professionals in the South East of Hampshire.
As a non case-holding team we do not receive referrals but we receive requests for clinical guidance and education.
The South East Community Diabetes service consists of  Diabetes Specialist nurses, Coordinators, Desmond lay Educators and Administrators . 
We also work closely with Portsmouth Hospitals Trust.

What do we do?

We run courses for people with type 2 diabetes across the South East of Hampshire. These courses can help you to learn more about your condition and manage it more effectively. It’s also a great opportunity to meet other people with diabetes and to ask questions or voice concerns.           
Desmond

Diabetes Education Self Management On Going for Newly Diagnosed, is aimed at people in the first year of diagnosis of type 2 diabetes. 

Desmond is a 1 day course which helps you find out more about type 2 diabetes, activities and lifestyle adjustments you can follow help to manage your condition.  

What’s involved?

 Usually a small group of people with their partner or family member or friend held at a local venue over one day.

The day is led by our team who are trained to deliver you with honest, up to date,             evidence based information to give you options to manage your diabetes.

Why go?

There is a lot of conflicting, out-of-date information available. This is your opportunity to acquire an understanding of Type 2 Diabetes and a chance to discuss and explore factors such as food choices, activity and medication.

 How can I attend?

Type 2 diabetes refresher sessions

We provide a 4 hour course aimed at people with type 2 diabetes who have either been diagnosed for longer that 12 months or feel they need fresh input into managing their condition.

 We have a range of educational courses and support for health care professionals, take a look a the options below to find out more.

The South East Hampshire  and West Hampshire Community teams together with the LEaD training department offer diabetes care and education to healthcare professionals across Hampshire at a variety of locations. We offer a varied portfolio of education for patients/ service users and healthcare practitioners. Wherever possible we aim to support patients/ service users to improve self care.

For staff not employed by Southern Health there may be a cost associated with some courses. External applicants will have to complete an external application form.

We also provide a 3 hour basic introduction to diabetes to staff to help them  support, manage and care for their residents with diabetes.

IMIT is aimed at Health Care Professionals, GPs, Drs and nurses who have already completed some diabetes training and are going to be managing and initiating insulin and other injectable therapies.
