What do we do?
We run courses for people with Type  2 diabetes across North Hampshire. These courses can help you to learn more about your condition and manage it more effectively. It’s also a great opportunity to meet other people with diabetes and to ask questions or voice concerns.

Who is this pathway for?

Adults diagnosed with type 2 diabetes in the previous 12 months.    

What will this cover?   

You will learn what it means to have type 2 diabetes, what tests there are, how to set targets and goals and also complications you may come into contact with.

You will be taught different lifestyle modifications and self-management techniques (e.g. checking your feet on a regular basis).

How to refer?

To be referred to this service please use our self-referral form and email it to . Alternatively you can request a referral through your GP, or other healthcare professional.
