Care Home Specialist Nursing

Our care home specialist nurses provide focussed, expert care to support patients in nursing and residential care homes.

Who is this service for?

We help patients who live in nursing and residential care homes who may be at risk of becoming more unwell. We want to ensure that worsening health problems don’t impact on your quality of life, or result in you being admitted to hospital.

Contact your GP to request a referral to this service, or speak to a member of staff at your residential care or nursing home.

How we can help

We can help by assessing your needs and carrying out diagnosis of health problems or conditions. We can then organise and provide medical and/or nursing care to help you. 

We work closely with your care or nursing home, as well as other agencies to make sure you receive the right types of care. If we feel that you may benefit from the support of another specialist service (such as a mental health team) we can make a referral on your behalf. 

We provide educational support for care and nursing home staff to help improve the care that all residents receive. We also provide education for patients, families and carers, such as advice on lifestyle changes that may improve your health and wellbeing.
