The BBC were interested in the new ways we are working in that area and some of the Extended Primary Care initiatives we have been trialing.
They filmed our Complex Care Team working with a patient who they had supported to better understand his conditions whilst also addressing his social isolation. The team uses a range of clinicians with different specialties and training to take a more holistic approach to care, addressing specific needs attached to clinical conditions as well as looking mental health support, education and social wellbeing.
Following this they visited our Long Term Conditions (LTC) Hub. The LTC is helping patients who have long term conditions by ensuring that the specialist nursing care they need to manage their conditions, such as diabetes, are available in one place. For the patient this removes the need to book several different appointments, making it more convenient. For staff it means they have specialist colleagues on hand to discuss treatment options and second opinions.
