Publication of independent homicide review by NHS England

27 September 2018

Today NHS England has published a quality assurance review into the care and treatment of a mental health service user (JK) in Southampton.

The independent investigation was commissioned to review the care and treatment provided to JK by Southern Health up to the time of the homicide in June 2014.

The full report can be read here.

The independent homicide review report identifies a number of areas for improvement in the care we provided and these largely mirror the findings of the Trust’s own investigation in 2015. Since that time we have implemented a number of significant changes and improvements to our mental health services in Southampton and are in the process of putting in place the additional recommendations highlighted in this report.

Karl Marlowe, Medical Director for Southern Health NHS Foundation Trust said: “We welcome this report and the opportunity for further assurance of our internal processes and developments to services. We have supported NHS England in compiling this report, and the hard work of our teams has been rightly recognised in meeting the majority of recommendations made.

"This report came from a terrible incident where a man lost his life in 2014, and we hope that the report provides his family and friends some confidence that improvements have been made to our services as a result.”
