Statement from the Trust regarding the death of James Younghusband in 2014

In August 2016, the NHS Litigation Authority, acting on the Trust’s behalf,  settled a claim with the family of James Younghusband, who tragically died at Ravenswood medium secure unit in 2014. The family of Mr Younghusband  have asked that a statement about his death be made public by the Trust. The Trust would like to reiterate its apology to the family that James tragically died whilst an inpatient. The Trust’s own internal review of the incident and a subsequent external review by HASCAS both identified a number of failings. An investigation was also undertaken by the Health & Safety Executive which found that the Trust had committed a breach with regards to its duty of care towards James but that it was not clear whether the outcome would have been different but for the deficiencies identified.  Significant  measures have been taken since James’ death including a £2m refurbishment programme at Ravenswood aimed at reducing ligature risks associated with the structure of the building.  By taking this action, we believe we have significantly reduced the risk of a similar tragedy occurring.
